It has truly been an honor to provide care for all my Portland friends and neighbors, but I have decided to move back to the east coast to be closer to family.

IF you are a Care Oregon member, you should have already been re-assigned (by OHP/Care Oregon) to a new primary care provider. Call Care Oregon (503-416-4100) to inquire about your new PCP. All records requests will be responded to within 14 days. I apologize if this news is a surprise to you. Letters detailing this information were sent to all patients approximately 3 months ago, but but I understand that people move and may not update their info with me or OHP. If you have urgent concerns, you can go to Urgent Care (Legacy Go Health or Providence Urgent Care). Understand that these types of providers may not be comfortable issuing refills for controlled substances, so establishing with a new PCP is the best course of action in that scenario.
