Cancellation/No-show/Tardiness Policy:

In the event you find that you are unable to make your appointment, please call at least 48 hours in advance to change or cancel so that we may make that appointment available to other patients in need of care. All patients that do not cancel within this time period are subject to a $100 late cancellation fee.

If a patient is more than 10 minutes late for a scheduled appointment, they will be listed as a no show and will need to reschedule their appointment. No shows are also subject to a $100 fee. 

If a patient cancels three consecutive appointments within 24 hours of their appointment; or no-shows two times within a 12 month rolling period; the patient will not be scheduled again, until they speak with the Clinic Administrator. The Clinic Administrator will work with the patient’s physician to decide if the patient may return to the practice. The Clinic Administrator will notify the patient of the final decision. If the patient is discharged from the practice a letter will be sent to the patient by certified mail.
